Solving Real-World Health Challenges
We collaborate with partners to develop and pilot innovative care models that address real-world challenges. Through testing and evaluation, we identify what works, refine promising practices, and create sustainable frameworks that deliver measurable improvements in health outcomes across the Carolinas. We identify barriers to implementation, regulatory challenges, and specific associated payment and policy changes needed for success.
Active Projects:
Comprehensive Palliative Care for Serious Illness Pilots
CaroNova is supporting implementation of a comprehensive care model and payment infrastructure to support people who are seriously ill, including those facing the end of their lives, and their families. Through pilots in the Carolinas, learnings will inform core elements for the CPCSI model, expected patient outcomes, sustainable payment models, and a framework for collaboration between providers and payors.
Optimizing South Carolina’s Behavioral Healthcare Delivery Infrastructure
CaroNova is working with the South Carolina Department of Health and Human Services (SCDHHS) and state partners to realize a behavioral health system that meets the basic and comprehensive needs of South Carolinians.
Designing the Innovation in Behavioral Health Model
CaroNova is partnering with the South Carolina Department of Health and Human Services (SCDHHS) as a neutral third-party convener to support implementation of the CMS Innovation in Behavioral Health (IBH) Model, a multi-year federal initiative designed to strengthen how care is delivered and paid for to support people with behavioral health needs, including substance use disorders.
Universal Youth Mental Wellness Supports South Carolina and North Carolina
Using the Mental Wellness Through Schools (MWTS) Framework, CaroNova and its partners are simplifying, strengthening, and standardizing the foundational elements needed to support youth mental wellness through collaborations with South Carolina and North Carolina.

Healthcare System Workforce Experience Accelerator
CaroNova is supporting a bi-state collaborative to transform the workforce experience, examining the root causes of burnout and lack of professional fulfillment, and working with health systems to codesign and launch an online resource suite of existing best practices and innovative strategies.
Completed Projects:
Opioid Stewardship Accelerator
CaroNova launched the Opioid Stewardship Accelerator (OSA) in 2022 to support hospitals and health systems responding to the devastating opioid epidemic that persists across the Carolinas. The OSA is ultimately intended to catalyze the communitywide collaboration and systems changes necessary to improve health outcomes.
Home Hospital Accelerator
Born out of opportunity during the COVID-19 pandemic, the Home Hospital Early Adopters Accelerator (HHEAA) supported hospitals and health systems to provide healthcare services in locations other than hospitals, making a more comfortable and less disruptive experience for low-acuity patients and their daily routines while also preserving hospital beds and staff for higher-acuity patients.
