Designing the Innovation in Behavioral Health Model

CaroNova is partnering with the South Carolina Department of Health and Human Services (SCDHHS) as a neutral third-party convener to support implementation of the CMS Innovation in Behavioral Health (IBH) Model, a multi-year federal initiative designed to strengthen how care is delivered and paid for to support people with behavioral health needs, including substance use disorders.

The model integrates behavioral health, physical health, and upstream drivers of health through value-based payment and coordinated care. South Carolina is one of only three states selected to participate in the first round of funding. The IBH model is unique because integrated care is provided through the behavioral health provider.

Where We Are

CaroNova is helping SCDHHS build the foundation for IBH implementation by:

  • Facilitating the design of the statewide care delivery framework and value-based payment approach (Medicaid Payment Approach)
  • Establishing and facilitating the IBH convening structure, including the Advisory Committee and workgroups
  • Developing the data, quality, and health IT strategy needed to support reporting and evaluation
  • Translating CMS requirements into practical, implementable plans

This phase focuses on thoughtful design, strong partnerships, and ensuring the model works in real-world clinical settings.

Where We're Going

Over the coming years, this work will support:

  • Implementation of the care delivery framework and Medicaid payment approach across participating practices
  • Strengthened health IT systems and data sharing infrastructure
  • Measurable improvements in care coordination, access, equity, and outcomes
  • A statewide model that serves as a national example of behavioral health transformation
