Prioritizing the Patient
CaroNova is supporting implementation of a comprehensive care model and payment infrastructure to support people who are seriously ill, including those facing the end of their lives, and their families. Through pilots in the Carolinas, learnings will inform core elements for the CPCSI model, expected patient outcomes, sustainable payment models, and a framework for collaboration between providers and payors.
Where We Are
Leveraging a value-based payment framework, we are piloting an approach to implement and validate a scalable way of delivering and financing comprehensive serious illness care. The CPCSI model centers on:
- engaging patients who meet defined eligibility criteria related to disease state, health care utilization, and functional status;
- ensuring that an interdisciplinary care team is in place to provide comprehensive palliative care services to patients and their caregivers; and
- using data to ensure that patient, caregiver, and provider experience of care improves and to determine the value generated from the model.
These collaborative partnership pilots are being conducted in South Carolina (with MUSC Health and BlueCross BlueShield of South Carolina) and North Carolina (with Carolina Caring and Piedmont HealthCare) to refine the CPCSI model and determine a payment methodology that allows for payor-agnostic scalability.
Where We're Going
- CaroNova aims to introduce additional payor partners to both pilots in 2026, increasing patient volumes and positioning both sites for sustainable funding.
- CaroNova is currently researching palliative care Medicaid payment designs and implementation successes from other states to leverage those learnings for the Carolinas.
- CaroNova is engaging partners in both states to better prepare the health workforce to engage and support the serious illness population.
- CaroNova plans to launch a serious illness bi-state learning collaborative in 2026, including providers, payors, and other stakeholders.
